Mary introduced Helen Lyndon, a Nurse consultant for older people who is attending to present and discuss her recent Audit of Frail Patients Admitted Frequently to RCHT. Action: Richard to circulate Audit notes document.
Helen outlined the position of the STP (Sustainability and Transformation Plan) in bringing together organisations and looking to address the huge deficit. This approach has been nationally mandated by NHS England.
Helen highlighted that 10 days in a hospital bed is the equivalent to 10 years of muscle aging in someone with frailty.
Helen also highlighted that 68 patients accounted for 400 admissions in a single year. 40% of these patients died in that year.
David highlighted the point that sometimes from Dom Care, patients are admitted and lost in the system. Often they get placed with another Dom Care agency, when the original agency has kept a slot open for that client and havent been informed of their discharge or other placement. Helen acknowledged how this could be an issue and offered to feed this back.
David invited Helen back after Christmas for an update on how the STP is progressing.
Mary presented some advice and questions that families should be asking about RAS. Action: Richard to circulate advice and questions about RAS for families.
A discussion took place around RAS. All RAS assessments now go to a weekly panel in order to check for consistency.
It was reported that Skylakes have still not finished their assessments. All new placements are being assessed by an in-house team rather than Skylakes.
The meeting also discussed fees owed as many providers still have outstanding debt from a number of sources. Action: Richard to meet with legal debt collection business based in PIC.
